Medication Policy:
A medication authorization form must be signed by a physician for students to take medication in school- this includes prescription and over-the-counter medications. Medication forms can be found below (medication authorization form) or can be picked up by your child in the nurse’s office. 
Routine Screening: :
A Physical is required for all 9th graders and it needs to be dated from June 1st of the student’s 8th grade year to May 31st of the student’s 9th grade school year.  A Physical/Sports Physical  is required to be on file in the nursing office in order for your child to participate in athletics. We suggest that you make a copy of your child’s physical prior to your child dropping off the physical so that you can have a copy for your own records.   These Physicals are valid for 13 months and need to be kept on file in order for your child to continue to participate in his or her chosen sport.  Mandatory scoliosis screening is done for male students in Grade 9.
