Trumbull Public Schools

Each pupil, before initial enrollment in the Trumbull Public Schools shall have had a health assessment within the preceding year if transferring from within the State of Connecticut. Each student transferring from out of the State of
Connecticut shall produce a cumulative health record or certified true copy from the previous school district. In either case, such records, to include a new physical examination, must be provided within 30 days of enrollment in order to continue in the Trumbull Public Schools. The results of the physical examination must be recorded on the State of Connecticut Department of Education Health Assessment Record Form.

Each pupil enrolled in Trumbull Public Schools shall have a health assessment at least once in each three year interval: in grade three, in grade six, and in grade nine.
